Brief Description
Issuance of Solo Parent ID to the qualified applicants to avail benefits and other privileges for solo parent

Documentary Requirements:

New Applicant

  1. Accomplished Application Form
  2. 1 pc. Recent 2×2 ID picture (white background)
  3. Original Barangay Certificate (Solo Parent)
  4. Birth Certificate of minor child/children (Xerox copy)
  5. Medical Certificate/PWD ID (if spouse or child/children is PWD)
  6. Any of the following:
  7. Photo copy of Death Certificate of spouse (if widow/widower)
  8. Affidavit (Circumstances of Being a Solo Parent)
  9. Copy of filed /Pending Petition for annulment of Marriage /Legal Separation
  10. Copy of Certificate of Detention/Court Order (if spouse/partner is in prison)
  11. DSWD Foster Care license (for Foster Parent)

For Legal Guardian

  1. Accomplished Application Form
  2. Original Barangay Certificate (Solo Parent)
  3. 1 pc. Recent 2×2 ID picture
  4. Birth Certificate of minor child/children (Xerox copy)
  5. Birth Certificate of minor’s parent / guardian (Xerox copy)
  6. Barangay Blotter
  7. Affidavit of Guardianship
  8. Any of the following:
  9. Death Certificate of minor’s parent/s
  10. Copy of Certificate of Detention/Court Order (if minor’s parent/s is in jail)

PROCESS

  1. Register to website: qceservices.liboph777.vip using your personal email address
  2. Fill out the form properly to register and submit
  3. Accomplish Sectoral Information. Make sure to fill-out all the details in this section.
  4. Upload Documentary Requirements for Solo Parents
  5. The application will be reviewed by Solo Parent approvers/social workers
  6. Wait for an email to be sent regarding the Solo Parent ID Application. Applicant will be receiving an email to schedule an appointment for social worker’s interview and assessment.
  7. Make online appointment @ the qceservices.liboph777.vip website, SSDD page. Present the reference number for verification of the schedule
  8. Come to SSDD (2nd Floor), Solo Parent Section on your appointment date & time for your interview & assessment
  9. Once approved the application, install the QCitizen Application in Playstore / Appstore to get the Digital ID.
